No luck in the attempt to recreate this in either 4.2 or the 4.3 test, I tried 2x times on each.  It might have just be a line misalignment or corruption on the save file.  I get these occasionally when I use the steam download, or if I use morghs while in game playing.

He means your King, I think he specifically means Valarr Blackfyre, does not accuse you of treason and take all of your lands if you have a low relationship status with him.

Hate to say it but that does seem about right from a loot standpoint for that force ratio.  What does your morale status state?  If it is excellent then you may be at the point of no gain.  Though it is rare sometimes you can max out at a lower level if you do not have a good food mix.  I have been known to butcher the prisoners for meat and party morale never improves much if you do that.

Was the  battle a reinforcement, protection of a caravan, or a straight on battle?  I myself have run into the bug, most of the time it is a failure of the text to display in the bottom left of the screen.  Double check the Recent Messages in information menu you might be able to catch site of battle rewards that way to confirm. The prisoner thing is something I have yet to experience but I am willing to see if I can recreate it. 

Additionally, what is your character's religion, origin culture?  What side did you take for the Red Wedding? I will see if I can recreate exactly what you experiencing.

What was your party size? Party size rapidly degrades the amount of loot you can acquire since the loot is split between you and you men. 

You have to be a freerider and you can get the quest from the guild master.  Just a heads up on the missions from the guild master:

Ale delivery- Once a Month

Kidnapped Daughter- Once a Month

Troublesome Bandits- Once a Month

Making Peace- Once a Month (You need to be a free rider and not attached to any faction)

Escorting Caravan- Always

Driving Cattle- Always

Trackdown Bandits- Always but only after attacks by bandits on peasant or a caravan.

Crownland's troops are only available to you if you choose to be Valyrian.  Capturing lands from Dany or taking the Crownland lands (Kings landing, Dukansdale, Rosby, Darry, Antlers, Driftmark, Dragonstone, and etc. does not unlock this troop tree to you is possibly a better explanation to this.

The technique I use is very simple.  I make peace with the Freefolk and then do quest from Thenn, Frostfangs Camp, and Castle Black.  Eventually, you will get the quest to make peace between the Freefolk and Nights Watch, though that can be a real pain to get the mission to come up.  This method does have one useful advantage as both the Freefolk and Nights Watch can be reinforced and preserve their combat power to aid in your fight against the White Walker Invasion.

This is true, though I have encountered a bug that causes generic troops (Common Faction) to only be offered.  This can be rectified by talking to your Master of Whispers about domestic policy and setting your culture.  Additionally, you can change your culture based on lands you have conquered with the exception of the Crownlands troops.  You have to be Crownlands/ Valyrian to choose this option.  One of the issues you will run into is the establishment of your companions as lords, their initial army setup will be based on the culture to which territory they have been assigned, however; they will recruit troops from your selected culture.  Hope this is of help to you!

Bad news... nope.  I have heard whispers that you might be able to accomplish it when the White Walker Horde arrives, but I am not 100% certain of that.  If anyone can confirm this please post.

All look like they have been in a losing match with a vacuum cleaner, or have been transformed into vampires.  On Morgh's editor all three characters have alphabetic values in some of their skill slots.  Just went into the troop file manually and all three of these characters are misaligned:
